I have obtained an establishment number from the United States Environmental Protection Agency(EPA).
I report the amount of pesticide sold/repackaged each year to the United States Environmental Protection Agency.
All measurement equipment (scales or meters) used for the repackaging of bulk pesticides have been certified by the SD Department of Commerce and Regulation.
I only repackage pesticides in containers allowable under the terms of the letter of authorization (Bulk Repackaging Agreement) from the registrant of the pesticide.
All of my bulk pesticide storage containers prominently bear the registered product label in the proximity of the point of dispensing.
All of the containers that I use to repackage bulk pesticides prominently bear the product label. That label has not been altered in any manner form that which the registrant of the product has provided except to add the EPA Establishment number assigned to my company.
I do not store or repackage bulk pesticides in such a manner so as to allow them to commingle with materials that are intended to be consumed by humans or animals.
Off loading and repackaging of bulk pesticides is conducted within an Operational Area Containment.
Has the container you are about to fill been inspected to be sure it is suitable for the product you are going to place in it, is it in a good state of repair that all gaskets, pumps, and fittings are in good working order to prevent leaks and spills?
If the container is not sealed and dedicated to a product or active ingredient, has it been properly cleaned according to clean-out procedures provided by the product registrant or manufacturer?
For containers that are to remain dedicated to a single product or active ingredient, are tamper-evident seals in place each time the container is refilled to prevent introduction of any other product without your knowledge?
Have you recorded the quantity(gallons or pounds) dispensed into the container on its product label when filling is completed?
Have you properly logged in the record book the amount of product transferred to the container you are filling?
